We don't know what looks like until we make it, and we don't make it until you tell us what you want.  What it is is a trapeze bracket with a custom hole pattern to fit your fancy or your unique guitar.  Tell us the hole pattern, we drill it to match, and Bob's your uncle. 
 
It's compatible with all the trapeze tailpieces that we sell (6-string, 12-string, long, bass, etc.), as well as the Kaufmann vibrola.  And yes, the mounting screws are included.
